Let’s Be Clear
Let’s Be Clear is a collaborative initiative by the South Dakota Department of Health and the Department of Social Services, created as part of the South Dakota Opioid Use Strategic Plan.
Its mission is to increase awareness, reduce stigma, and provide resources for the prevention, treatment, and recovery of substance misuse and opioid use disorder across the state. By fostering open conversations and dispelling misconceptions, Let’s Be Clear aims to create moments of clarity that inspire hope, understanding, and action toward recovery.

Care Coordination
Those struggling with addiction can feel isolated and hopeless. A Care Coordinator provides the support you or your family may be missing and helps develop a plan for recovery.
Care Coordination is a FREE, confidential service available for all South Dakotans.
How to Start:
- Call 988: The 988 Lifeline can get you in touch with a Care Coordinator.
- Walk Through Options: A Care Coordinator will go through options and schedule times that work for you.
- Sign Up Online: You can always sign up online by filling out the form on the learn more page.
The Link
As a community triage center, The Link will be a safe place for people experiencing a non-violent behavioral health crisis or needing care for substance abuse disorders to access immediate treatment and referral to support services.
This facility is a collaboration of the City of Sioux Falls, Minnehaha County, Avera Health and Sanford Health. It offers our community an innovative approach to address gaps in critical services for people living with mental health and substance abuse challenges.

Syndicate 12
Syndicate 12 is a cutting-edge social platform designed to support individuals in recovery and their loved ones. Through Syndicate 12, users can create intimate groups of up to 12 people to form a strong support system. This online space has a wealth of resources, including days in recovery tracking, educational content, influencer engagement, group meeting spaces, health and recovery articles, and gamification with rewards.
Users can earn points on the platform for various positive actions such as signing up, reading articles, watching videos, taking surveys, creating popular posts, and creating groups. The group meeting spaces also allow users to participate in virtual support groups with video chat capabilities.
The gamification element of Syndicate 12 is designed to help users reestablish their dopamine reward system. Isolation is a common aspect of addiction, but Syndicate 12 is committed to creating meaningful connections within the community. Users can redeem points earned for positive behavior, such as days in recovery, for rewards donated by the local community, including coffee, care packages, meals, and other gifts.
